Tirzepatide reaches this list in two ways. 12 of the 46 providers prescribe a brand product from its maker, and 41 prescribe a version a pharmacy compounds to order. 7 offer both.
The brand side has a label. FDA approved Mounjaro for type 2 diabetes on May 13, 2022, and Zepbound for chronic weight management on November 8, 2023. Zepbound added obstructive sleep apnea in adults with obesity on December 20, 2024. Mounjaro added a heart-risk indication for people with type 2 diabetes on August 27, 2026.
Every approved tirzepatide is an injection. So the topical, capsule, tablet, sublingual and dissolving tablet forms that 8 providers sell here can only be compounded, with no FDA label for that form.
FDA first called the tirzepatide shortage resolved on October 2, 2024. A lawsuit sent that decision back to the agency, and FDA reached the same finding again on December 19, 2024.
The shortage-era room for compounding then closed. For 503A pharmacies it ended when a court denied relief on March 5, 2025, as FDA's update reported. For 503B outsourcing facilities it ran to March 19, 2025.
What remains is narrower. An individual prescription alone does not remove the restrictions on essentially-copy products. FDA reminded compounders on April 1, 2026 that tirzepatide is not on its shortage list. FDA also proposed, on April 30, 2026, keeping it off the bulk list outsourcing facilities draw from. No final decision had appeared as of this update.
A compounded tirzepatide isn't reviewed by FDA for safety, effectiveness or quality. Through May 31, 2026, FDA had more than 730 adverse-event reports tied to compounded tirzepatide. Reports do not establish causation or an incidence rate.
Licensed US pharmacy. This chip is where most tirzepatide providers lose their A. 26 of the 46 don't name a dispensing pharmacy with an active license, and that one Flag explains 26 of the 41 B grades. Without a pharmacy name, you can't ask who made the vial.
Prescriber reviews you first. Each of the 46 says a clinician looks at your intake before writing a prescription. Zepbound and Mounjaro are prescription drugs, and so is every compounded copy, so this step is not optional anywhere.
Price shown before signup. 19 print a month-to-month figure before they ask for an email; AHBARx, Belle, bmiMD, Bodybuilding Health+ and 23 more don't. An "as low as" rate that needs a 6- or 12-month prepay isn't that figure.
No research-only products. Tirzepatide is a prescription drug, so offering it never costs this Pass. A Flag here means another catalog product needs regulatory clarification. That applies to DirectMeds, He & She MD and Pepti.
The grades split 5 A to 27 B. One Flag separates the two, so read which chip a B lost before you rule it out; a missing pharmacy name is a question one email can settle.
Open any name for the full check, quoted from that provider's own pages. The methodology lists what turns a Pass into a Flag.
No prescription step at all. Sites that sell tirzepatide "for research" ship it with no prescriber and no named pharmacy. Those sellers aren't graded here, because they don't sell care.
A newer peptide sold next to it. Retatrutide is an investigational drug with no FDA approval. FDA says it can't legally be used in compounding, and "research use only" retatrutide is an unapproved drug. A provider that offers it gets a Flag on the research-only check.
A form with no label behind it. Drops, capsules and creams of tirzepatide are sold, but no FDA review covers them. Ask the prescriber why that form, and ask the pharmacy what testing each batch gets.
The 17 standing figures on this list span $149 (Brightmeds) to $433.44 (iThriveMD). At the median of $249.99, a year comes to $2,999.88; at the floor, $1,788.
Compounded-only providers sit around $225 a month at the median. 2 brand-only providers print figures from $299 to $364, and each check page names the brand product behind the figure.
The lowest A-graded figure is SnagRx at $149, which is $0 a year above the floor.
Prepaid plans aren't used here. A provider may print a lower monthly rate for 3, 6 or 12 months paid up front, so ask what you'd lose if the prescriber stops the drug early.
Watch the billing period: 4 providers charge per 4 weeks or 28 days, at 28-day intervals, and the cards convert that to a calendar month.
Zepbound, Mounjaro or compounded tirzepatide? Each has a different price, a different maker and, for the compounded one, no FDA label.
26 of the 46 providers leave the pharmacy unnamed on their pages. Ask for the pharmacy and its licensing state, and whether it is a 503A pharmacy or a 503B facility.
5 of the 46 providers show a lower first-period figure. Ask what the standing price is, and whether it changes when your prescriber changes the dose.
A provider that also sells retatrutide or other research-use peptides shows a Flag on that check. Read what it sells before you sign up.
