What the fine print shows
Heva Health is an Arizona-rooted telehealth practice (the site says Heva Health and the Dose clinic in Phoenix are one medical practice) that sells GLP-1s, hormone programs and a small longevity shelf. The longevity products are compounded NAD+ in three forms (a product labeled simply "NAD+", a nasal spray and an NAD+ cream), a glutathione injection vial and a PT-141 vial. Each is a one-time purchase marked "No recurring charges", and none states how long it lasts. That's why the table below can't turn them into a monthly cost. Ask your provider how long one order should last before you compare it with a monthly plan elsewhere.
The pharmacy side is named, which is uncommon. The how-it-works FAQ says Heva works with Strive Pharmacy and Stryker Compounding Pharmacy. That openness lets the license check run, and it is also why the check is a Flag: the checked Strive San Antonio Texas facility shows probation; this is not the status of every Strive branch. Stryker Compounding Pharmacy didn't turn up under that spelling on the boards searched, so its line stays unsettled. Ask which of the two will fill your order, and from which state, before it ships. The dated records below are scoped to the specific facility and jurisdiction checked.
A licensed provider reviews your 60-second intake and messages a recommendation, usually within a day; a few treatments need a short video call. The clinical team on the product pages is a mix: the chief clinical officer is a naturopathic doctor (NMD), alongside a DNP, a PA-C and a family nurse practitioner. If you want a medical doctor specifically, ask who will review your file. If the provider decides a treatment isn't appropriate, Heva refunds the whole program, visit and labs included, as long as the order hasn't reached the pharmacy.
For GLP-1 shoppers, month-to-month compounded semaglutide is $199/mo and tirzepatide is $399/mo; the "from" figures on the shop cards are 6-month program rates. Compounded tirzepatide isn't dispensed in Alaska, Arkansas, Indiana or South Carolina. The NAD+ page and the glutathione page cover where those compounds stand with FDA.
The refund cutoff is more precise than delivery. Current terms define pharmacy fulfillment as transmission and acceptance by the compounding pharmacy for preparation. They permit a full refund of program fees, consultation and labs for cancellation before that point, as well as for clinical unsuitability before fulfillment. Ask support to confirm whether transmission has occurred rather than waiting for tracking. Product forms and state eligibility also need separate confirmation. A nasal spray, cream or unlabeled NAD+ form is not clinically interchangeable with an injection, and a wellness claim does not prove benefit from the dispensed preparation.

How signing up works
1Sign up
You add a product to your cart or take the short intake, which asks about your goals, symptoms and medical history. Payment is by card, HSA or FSA; Heva doesn't bill insurance.
2Clinician review
A licensed Heva clinician reviews your intake and messages a recommendation, usually within a day. A few treatments need a short video call first.
3Pharmacy
If you're prescribed, Strive Pharmacy or Stryker Compounding Pharmacy prepares the order, and a licensed pharmacist verifies it. Brand GLP-1s go through the maker's own pharmacy program instead.
4Delivery
The company describes shipment within a few business days and dashboard plan changes. A dashboard cancellation cannot be assumed to undo a prescription already accepted for pharmacy preparation.
Pharmacy and prescriber
- Strive Pharmacy:Texas license 34585: probation; prior disciplinary orders listed. This is the Texas record; confirm the assigned facility and other relevant states.Board license record
- Stryker Compounding Pharmacy:not yet matched to a state board record
- Prescriber step:A licensed clinician looks over your intake before anything is sentPass
What holds up
- The how-it-works FAQ names both compounding pharmacies: Strive Pharmacy and Stryker Compounding Pharmacy
- Heva refunds the full program, visit and labs included, if a provider declines before pharmacy fulfillment
- NAD+ comes as a nasal spray and a cream as well as the standard product, each with its price on the shop page
- Each NAD+ and glutathione order is a single charge with no auto-renewal to cancel
- The product pages show each clinician's credential, so you can see who practices there
Watch-outs
- The checked Strive San Antonio Texas facility shows probation; confirm the actual dispenser before paying
- NAD+, glutathione and PT-141 orders state no supply length, so you can't tell the monthly cost
- The chief clinical officer is a naturopathic doctor; ask for an MD or NP if that matters to you
- Refund cutoff is pharmacy transmission and acceptance for preparation, which can precede shipment
- Compounded tirzepatide isn't dispensed in Alaska, Arkansas, Indiana or South Carolina
Fits you if
You want NAD+ in a nasal spray or cream, or a single glutathione vial, without committing to a subscription, and you value a practice that names the pharmacies it uses.
Look elsewhere if
You want a standing monthly price for NAD+ or glutathione, or you need the actual dispensing facility and current license checked before payment. Readers who want a physician-led team may prefer a provider whose prescribers are MDs.

Questions about Heva Health
- How much does NAD+ cost at Heva Health?
- The NAD+ shop page prints a one-time price for each form, with the cream the lowest. None states how long an order lasts, so there is no monthly figure; the table below shows the printed prices.
- Which pharmacy fills Heva Health orders?
- The how-it-works FAQ names Strive Pharmacy and Stryker Compounding Pharmacy. The checked Strive San Antonio Texas record shows probation, so ask which pharmacy will fill your order before you pay.
- Who prescribes at Heva Health?
- Licensed clinicians on Heva's team review every intake. The product pages list a naturopathic doctor as chief clinical officer, plus a DNP, a PA-C and a family nurse practitioner.
- Can I get a refund from Heva Health?
- Yes, if a provider decides a treatment isn't right for you before the order reaches the pharmacy. The refund covers the whole program, including the visit and labs. The terms also permit full program-fee refunds for cancellation before pharmacy fulfillment. Fulfillment means prescription transmission and acceptance for preparation, which can occur before shipment.
- Does Heva Health serve every state?
- The how-it-works page says it is available in all 50 states where licensed, but its fuller care path runs in only 12 states, and compounded tirzepatide isn't dispensed in Alaska, Arkansas, Indiana or South Carolina.
