Question

Does tesamorelin work? Evidence by outcome

Tesamorelin is one of the few growth-hormone peptides with large randomized trials behind it. They answer a narrower question than the one most buyers are asking.

The short answer

For its approved use, yes. In a 26-week trial of 412 adults with HIV and excess belly fat, tesamorelin cut deep abdominal fat 15.2% while placebo rose 5.0%. It didn't reduce fat under the skin or body weight, its label says it isn't a weight-loss drug, and benefit for general weight loss in healthy adults is not established.

Key takeaways

  • Visceral (deep belly) fat fell about 15% against placebo at 26 weeks in two randomized trials of adults with HIV.
  • People who stayed on it kept the reduction at 52 weeks. Fat under the skin of the belly didn't change meaningfully.
  • Both Egrifta labels say it is not for weight loss, because it is weight-neutral.
  • Selected liver, cognition and DXA body-composition trials report exploratory findings. They do not establish routine general weight loss, strength or longevity treatment.
  • A cognition trial in older adults also measured body composition; its secondary results do not establish general weight-loss or strength benefits.

Tesamorelin is the active ingredient in Egrifta, approved to reduce excess abdominal fat in adults with HIV-associated lipodystrophy [1]. Approved-use results should be separated from exploratory off-label findings. The overview explains the distinct formulations; this page compares clinical endpoints.

Short answer: established benefit for its approved population

"Works" depends on the goal. For shrinking visceral fat, the deep fat packed around the organs, in adults with HIV, the evidence is strong: two large randomized trials, replicated, with CT scans as the measure. A trial in older adults also measured body composition, but that does not establish a broad indication for weight loss, muscle function or slower aging.

The main trial: 412 adults, 26 weeks, CT scans

In the trial behind the approval, 412 adults with HIV and abdominal fat buildup, 86% of them men, took daily tesamorelin or placebo for 26 weeks [4]. Visceral fat on CT fell 15.2% on tesamorelin and rose 5.0% on placebo. Triglycerides dropped 50 mg/dL against a 9 mg/dL rise, and IGF-1, a blood marker of growth-hormone activity, rose 81%.

The trial found no significant differences in glucose or insulin. More people on tesamorelin dropped out because of an adverse event, though overall adverse events didn't differ between groups.

Two trials pooled, and what happened at 52 weeks

Across both phase 3 trials, 806 adults were randomized, two to tesamorelin for every one to placebo [5]. The treatment effect on visceral fat was −15.4% at 26 weeks. Fat under the skin of the belly barely moved: a −0.6% treatment effect that wasn't significant.

At 26 weeks, people on tesamorelin were randomized again, to keep going or switch to placebo. Those who stayed on it for 52 weeks kept a 17.5% reduction in visceral fat from their starting point, along with smaller waists and lower triglycerides [5]. The label tells prescribers to weigh continuing in people whose visceral fat hasn't gone down [1].

What it doesn't do: weight, and fat under the skin

Tesamorelin moves deep belly fat, not the scale. Both Egrifta labels say it "is not indicated for weight loss management as it has a weight neutral effect" [1] [2]. The pooled abdominal subcutaneous-fat difference was not statistically significant; that is not proof of zero effect in every person [5].

Does it work without exercise? The trials compared tesamorelin with placebo, not with an exercise program, so they don't answer that. They also excluded people with type 1 or type 2 diabetes and people with a history of cancer [1].

Liver fat: selected off-label trials

The liver-fat trial enrolled 61 people with HIV and fatty liver; 30 received tesamorelin and 30 placebo. The 12-month between-group hepatic-fat-fraction difference was −4.1 percentage points (95% CI −7.6 to −0.7; P=.018), corresponding to 37% relative reduction [6]. A separate 35% versus 4% reached the study’s below 5% threshold. Glucose and HbA1c changes did not differ.

This is not the only liver-fat trial. Stanley’s earlier 2014 study randomized 50 adults with HIV and abdominal fat accumulation;48 received treatment. At 6 months, median hepatic lipid-to-water percentage changed −2.0% with tesamorelin versus +0.9% with placebo, a net −2.9 percentage-point difference (P=.003) [8]. The endpoint differs from 2019’s hepatic-fat fraction. Both selected populations and follow-up durations limit generalization; neither trial creates a general liver indication.

Memory and thinking in older adults: one exploratory trial

In 152 adults aged 55 to 87, about 40% with mild cognitive impairment, 20 weeks of tesamorelin improved a combined memory-and-thinking score compared with placebo (P=.03), mostly through executive function [7]. Body fat fell 7.4% and DXA lean mass rose 3.7%, without a body-weight difference. DXA lean mass does not establish stronger muscles. Fasting insulin rose 35% in the group with mild cognitive impairment, and 68% on tesamorelin reported side effects, mostly mild, against 36% on placebo.

The cognition trial does not establish a routine memory treatment. The authors note small individual-test changes and uncertain clinical importance because functional status was not measured. NIH/VA supported the study; the manufacturer supplied tesamorelin and placebo [7].

Body composition in older adults: a secondary finding

The cognition trial included healthy older adults and people with mild cognitive impairment; 137 of 152 randomized participants completed it. Its body-composition findings therefore cannot be dismissed as HIV-only evidence [7]. They also do not demonstrate better muscle strength, general weight loss or longer life. The exact endpoint, population and treatment period still define what the result supports.

How it compares with other growth-hormone peptides

Tesamorelin and sermorelin should not be treated as interchangeable. Tesamorelin has large phase 3 trials in HIV-associated abdominal adiposity. Sermorelin also has controlled adult data, including a 31-person HIV-lipodystrophy trial, but different endpoints, populations and formulations prevent a reliable head-to-head ranking. The sermorelin evidence review explains those selected results.

The reviewed sources do not establish a direct head-to-head comparison of these medicines. The side-by-side pages, tesamorelin vs sermorelin and tesamorelin vs ipamorelin, set out what each has and lacks. The single-compound pages are the sermorelin overview and the ipamorelin overview.

Is it worth it? What you get against what the label warns

For someone with HIV and excess visceral fat, the trade is spelled out. On the benefit side: about 15% less visceral fat on CT, kept while treatment continues. On the risk side, the label lists these [1]:

  • Blood sugar: HbA1c reached 6.5% or higher in 5% of people on Egrifta and 1% on placebo by week 26.
  • IGF-1: at 26 weeks, 47% of people had IGF-1 more than 2 standard deviations above normal and 36% more than 3. The label calls for monitoring and says to consider stopping if levels stay high, particularly when the fat response is weak.
  • Injection-site reactions: 25% on Egrifta and 14% on placebo in the first 26 weeks.
  • Hypersensitivity reactions in 4% of treated patients, and fluid retention such as swelling, joint pain or carpal tunnel symptoms.
  • Cancer: not for people with an active malignancy, because growth hormone is a growth factor.

Outside the approved population, benefit and risk need their own evidence. The liver and cognition studies report selected positive outcomes, but do not establish routine use for weight loss, muscle function or longevity. Their safety observations also cannot simply be replaced with the HIV-trial rates. See the safety review.

How to get it legally

Egrifta is a prescription drug for its labeled use. A doctor may prescribe it off-label, which is the prescriber's call. Tesamorelin’s biologic status means ordinary 503A/503B compounding exemptions do not apply [3]. The status is on is tesamorelin FDA-approved, and practical label facts are in how tesamorelin is taken and whether tesamorelin needs refrigeration. How these pages pick sources is on how we check.

Frequently asked

How quickly does tesamorelin work?
The trials reported visceral fat at 26 weeks. The label says IGF-1 rose as early as 13 weeks, but fat results were measured at the 26-week scan.
Do I have to be asleep for tesamorelin to work?
The Egrifta labels do not establish sleep as a requirement for benefit. Follow the formulation-specific prescription instructions; a trial’s study schedule does not prove a universal optimal time.
Does tesamorelin cause weight loss?
Both labels describe a weight-neutral effect and say it is not indicated for weight-loss management. Reduced visceral fat in the studied HIV population does not establish general weight-loss treatment or guarantee no individual scale change.
Is tesamorelin better than sermorelin?
The reviewed evidence does not establish a head-to-head comparison. Tesamorelin has large trials for HIV-associated excess abdominal fat; sermorelin’s smaller adult trials use different populations and endpoints, so a numerical ranking would be misleading.
Is tesamorelin FDA-approved?
Yes, Egrifta has a specific HIV-associated lipodystrophy indication. Approved formulations differ. Ordinary503A/503Bcompounding exemptions do not apply to this biologic; that is separate from authorized investigational use.
